A. POLICY

A waiver determination and notice of the decision are required for every waiver request.

B. KINDS OF WAIVER DECISIONS

You can make three types of waiver determinations: waiver approval, waiver denial and split decision. Instructions for completing the appropriate notice for each type of waiver determination follow.
