DI 27530: Reopenings - Completion of Form SSA-831-U5

A. Disability Determination Services (DDS) authority and actions

The DDS may change a prior determination after applying the conditions for reopening and revision. The DDS must prepare a new Form SSA-831 Disability Determination and Transmittal to change a prior determination.
