Send the original copy of the initial SSA-101-U3 and any subsequent post-adjudicative actions to:

(This office makes the determination and certifies payments to applicants in all non-insured cases.)

In an initial case, if the VA Form 21-4180 is in file, send only the initial certification and any enclosures to the VA office shown on the form.
