Social Security Administration (SSA) domestic field offices accept Canadian issued marriage certificates, divorce decrees, and annulment decrees that meet the Social Security number (SSN) evidence review and foreign evidence guidelines as acceptable evidence of a legal name change.

We may accept additional Canadian documents as evidence of a legal name change, if approved by the Canadian Name Change Evidence Workgroup. Additional documents approved by the workgroup will appear in the legal name change drop down list, on the legal name change screen in the Enumeration System (SSNAP). Before accepting an approved Canadian name change document, compare the document’s descriptive features against the characteristics of a valid one, if available.

If the Canadian document submitted as evidence of a legal name change does not appear in the legal name change drop down list in SSNAP, consult with your regional office (RO) regarding the document’s acceptability. RO’s should submit acceptable Canadian documents requiring an assessment to the Canadian Name Change Evidence Workgroup for evaluation by email to ^OISP Foreign Documents.
